5.7. Forms

Forms (also known as Modals) allow you to collect specific information from users before creating a ticket. This helps your support team understand the issue better and respond more efficiently.

What are Forms?

Forms are interactive pop-ups that appear when a user clicks a ticket panel button. They can contain various input fields to gather information such as:

  • Issue description
  • Username/IGN
  • Order number
  • Priority level
  • Contact information
  • And more custom fields

Creating a Form

To create a form:

  1. Navigate to your server's dashboard
  2. Go to the Modals section in the sidebar
  3. Click "Create Modal"
  4. Configure your form fields
  5. Assign the form to a ticket panel

Form Fields

Forms support different types of input fields:

  • Short Text: Single-line text input for short answers
  • Paragraph: Multi-line text input for longer descriptions
  • Select Menu: Dropdown menu with predefined options
  • Number: Numeric input for quantities, IDs, etc.

Best Practices

Keep It Simple

Don't overwhelm users with too many fields. Ask only for essential information.

Clear Labels

Use descriptive field labels so users know exactly what information you need.

Optional vs Required

Mark fields as optional when the information isn't critical. This reduces friction for users.

Using Forms with Panels

Once you've created a form, you can assign it to any ticket panel. When users click the panel button, they'll see the form first, and the ticket will only be created after they submit the form with their information.
